The most common response an Indian woman receives when she tells her doctor she is always tired is a blood test that comes back normal. Her haemoglobin is 11.5. The doctor says it is a little low but fine. She goes home still tired, with no explanation and no plan. What was not tested was her ferritin. What was not addressed was the gap between a haemoglobin that looks acceptable and iron stores that have been running on empty for months.

Understanding the deficiency: haemoglobin versus ferritin

Haemoglobin measures the iron currently in red blood cells performing oxygen transport. Ferritin measures stored iron, the reserve the body draws from when dietary intake is insufficient. Ferritin depletes long before haemoglobin becomes abnormal.

A woman with a haemoglobin of 12 can have a ferritin of 8, which represents significantly depleted iron stores with real consequences for energy, cognitive function, hair health, and immune response. She does not have clinical anaemia by haemoglobin standards. She has iron deficiency by the measure that actually predicts symptoms.

A review by Jimenez et al. (Nutrients, 2010) established that iron deficiency without anaemia, defined as low ferritin with normal haemoglobin, is associated with fatigue, reduced exercise capacity, impaired cognitive performance, and hair loss. These symptoms resolve with iron repletion even when haemoglobin was never in the anaemic range. The threshold that most Indian laboratories flag as low, typically below 12 to 15 ng/mL, is now understood by many haematologists to be too conservative. Symptoms are frequently present at ferritin levels below 30 ng/mL.

If you are persistently fatigued and have not tested ferritin specifically, this is the single most useful blood test you can have. Request it by name. A standard complete blood count will not give you this value.

The biology of non-heme iron absorption

All plant-based iron is non-heme iron. It is absorbed through a different transporter than heme iron from meat, at a lower baseline efficiency of 2 to 20 percent depending on conditions (Hallberg and Hulthen, American Journal of Clinical Nutrition, 2000). Three factors determine where in that range your absorption falls.

Vitamin C presence is the most powerful enhancer. Ascorbic acid reduces ferric iron to ferrous iron at the point of absorption, enabling the divalent metal transporter that moves iron across the intestinal wall to do its job. The enhancement is dose-dependent: even 25 to 50 mg of Vitamin C alongside an iron-containing meal meaningfully increases absorption. At 100 to 200 mg, the effect is substantial. Amla, with its 600 to 800 mg of Vitamin C per 100g, is the most powerful iron absorption enhancer in the Indian pantry.

Tannin inhibition is the most important factor working against absorption. Tannins in tea and coffee bind to non-heme iron in the gut, forming insoluble complexes that cannot be absorbed. A study by Disler et al. (Gut, 1975) found that tea reduced iron absorption by 62 percent when consumed with a meal. Coffee reduced it by 39 percent. This single factor likely explains a significant proportion of the persistent iron deficiency in Indian women who eat reasonably iron-rich food but drink chai with or immediately after every meal. Shifting chai to 45 minutes after eating is one of the highest-leverage, lowest-effort nutritional changes available to iron-deficient Indian women.

Phytate inhibition from whole grains and legumes reduces iron absorption, but is substantially countered by soaking, sprouting, and fermentation, all traditional Indian food preparation practices. Soaking dal before cooking reduces phytate content by 30 to 70 percent. Fermenting batter for idli and dosa reduces phytates dramatically. These practices were not designed with iron absorption in mind but they solve the problem effectively.

The best Indian food sources of iron ranked by practical bioavailability

Sesame seeds (til) provide approximately 14.5 mg of iron per 100g. Combined with their fat matrix that supports fat-soluble cofactors and their co-presence with calcium and zinc, they are among the most nutritionally complete iron sources in the Indian diet. Til chutney, til ladoo, or sesame sprinkled on sabzi are practical daily additions.

Horse gram (kulthi dal) provides approximately 7 mg of iron per 100g and is significantly underused outside South Indian cooking. It is one of the most iron-dense legumes in the Indian repertoire. Rajgeera provides 7.6 mg of iron per 100g alongside complete protein and calcium. Methi leaves provide approximately 1.9 mg per 100g fresh weight but contain the additional benefit of galactomannan fibre that supports overall gut health and mineral absorption. Dark green leafy vegetables including spinach and coriander provide meaningful iron alongside Vitamin C, making them natural co-absorption partners.

The practical protocol

Test ferritin. If below 30 ng/mL, address it. Shift chai to 45 minutes after meals immediately. Add one Vitamin C-rich food to every iron-containing meal.
